Substitute Teacher Information

Lee/Ogle ROE maintains the personnel files for all substitutes as required by law.  Please bring in these documents when you apply to be a Lee/Ogle Substitute Teacher. 

  • a record of a physical examination - obtain from physician
  • results of a tuberculin test taken within a year of hiring or applying to be a substitute - obtain from physician
  • a criminal background check - obtain form from ROE
  • substitute, initial, or standard certificate
  • transcripts of college work
  • completed Employment Eligibility Verification (US Department of Justice form I-9) - Download here
  • completed mandated reporter form - (DCFS CANTS 22) Download here

When each substitute teacher’s file is complete, that person’s name will be distributed to the districts where (s)he plans to substitute.  Some districts maintain some or all of this information in files for substitute teachers who work in their districts.   

The following information may be helpful as you gather the required documentation: 

  • If you are a recent graduate who has not yet received your certificate, your entitlement papers from your university are enough to be placed on the substitute list when all other items have been submitted to the ROE.
  • Previous physical exams can be verified with a note from the doctor/office that completed the medical work.  Complete results of examinations are not necessary.  A copy of tuberculin test results no older than a year is sufficient.
  • Requests for criminal background checks will be sent from the ROE when you notify this office that you intend to substitute teach.  If you have already undergone such a check through our office, the record of that is sufficient.  If you completed a check through a school district office, ask the district to call the ROE for verification.  Effective August 12, 2004, all new substitutes (by law) need to have finger printing.  Instruction on how to do this is provided as each county differs on the method used.
  • If transcripts are not already on file at the ROE, you will need to provide these.
  • The I-9 and mandated reporter forms are available at the ROE.
  • The ROE will request verification of the required documents from any Lee/Ogle district where a substitute teacher was recently employed as a part-time or full-time teacher.
